NOMINATION OF GUARDIAN FOR MINOR CHILD
A Nomination of Guardian of Minor Child allows you to specify who you prefer to appoint as the guardian of the person and estate of your minor child should you predecease your child.
The nomination can address such concerns as:
1. Who will act as the guardian of your child. Who will be the person(s) who will raise your child in the event of your death.
2. Who will act as the guardian of your child's estate. This person can be the same person as the guardian of your child's person. Many times this job is split as one person may be more nurturing and another better with finances.
3. Provides that estate assets can be used for the health, education,
maintenance and support of your minor child, including items such as sports, dance classes and the like.
4. Request that health insurance be maintained for your minor child.
5. Request that your child be raised in a particular religion.
6. Permit the use of your residence for the guardian to live while raising your child.
7. Request contact with your family members and your children, if the guardian is not a family member.
8. Request any other information which may be important to you and your family in raising your child.
